HAPevolve Partners with Main Street Health to Provide Hospitals with a New Value-based Care Strategy

As an industry partner to HAP members, Main Street Health brings its unique model to hospitals in Pennsylvania’s 59 rural counties.
Main Street currently partners with over 900 primary care clinics – both hospital-owned and independent – across 20 states. The core of Main Street's model is placing a locally-hired Health Navigator (at no cost) in each clinic location to work alongside existing clinic staff to support their Medicare and Medicare Advantage patients. The Health Navigator uses Main Street’s proprietary technology and closed loop workflows to assist with care coordination and quality gap closure activities including preventive screenings, medication adherence, specialist referrals, and resolution of patient social determinant of health (SDOH) needs. This model has been successful in improving patient experience and driving over 40% improvement in quality Star scores. Additionally, Main Street Health pays its partners a predictable, transparent, monthly revenue stream with no downside risk or upfront investment.
