This privacy notice discloses the privacy practices for HAPevolve’s Portable Response Emergency Plan (“PREP”) at web address https://www.prephc.com (“Website”) and/or users of PREP’s mobile application (“Application”) (collectively referred to as “Services”). This privacy notice applies solely to information collected by this website. It will notify you of the following:

1. What personally identifiable information is collected from you through the website, how it is used, and with whom it may be shared.
2. What choices are available to you regarding the use of your information.
3. What security procedures in place to protect the misuse of your information.
4. How you can correct any inaccuracies in the information.
    

By using PREP or visiting or using the Website or Application, you are agreeing to the terms of this Privacy Policy and you expressly consent to our collection, use, and disclosure of your personal information in accordance with this Privacy Policy. This privacy Policy is incorporated into and subject to our Terms of Use (“Terms”). If you do not agree with the terms of this Privacy Policy, you must stop using the Services.

In this Privacy Policy, the term “Personal Information” means any information collected by us or provided by you that, with reasonable efforts, may be used to identify you, which may include first and last name, email address, telephone numbers.

 

Information Collection, Use, and Sharing

HAPevolve does not share any information you may provide us. We only collect information that you voluntarily give us via email, direct contact from you, or information you upload to the platform. We will not sell or rent this information to anyone.

PREP hosts user uploaded response plans that could contain address, telephone, or location information (i.e., written, images, floor plans, etc.). The PREP user is 100% in control of what information is uploaded to the platform, and can, at any time, adjust the information contained within their PREP.  Emergency plans should contain information that would provide facility’s staff with appropriate and facility-centric response to an emergency, and should not contain any confidential information.

We will use information you provide us in a few key ways.

• To provide you with Services
  • The information you provide will be used to populate the PREP, and to support you in training, onboarding, and in the general use of the product. For example, assisting in creating your plans and/or adding users, groups.
• To operate the Services and to improve and customize your experience
  • We will use information you provide to improve your experience using the service, and in customizing it to meet your specific needs
  • To provide you with support and handle requests, we use your contact information to provide any support you may require
• To send you updates, notices, announcements, and additional information related to the Services
  • Occasionally we have important news that we will communicate to you
• Subject to your prior indication of consent, we may also use the email address or mobile phone number that you provide when setting up your account to send you additional educational information and marketing material. At any time you may choose not to receive such materials by contacting us via email at Info@HAPevolve.com.
• To conduct surveys of your experience
• To enforce the Terms
• To contact you when HAPevolve believes it to be necessary
• To prevent misuse of the Services such as entering Protected Health Information.
  • *Note: Protected Health Information (PHI) or any confidential information should not be uploaded/imported onto PREP.
• To handle breakdowns and malfunctions

Your Access to and Control over Information

You may opt out of any future contacts from us at any time. You can do the following at any time by contacting us via the email address or phone number given on our website:

• See what information we have about you, if any.
• Change/correct any information we have about you.
• Have us delete any information we have about you.
• Express any concern you have about our use of your data. 

As stated above, the user is 100% in control of what information is uploaded to the platform, and can, at any time, adjust the information contained within their PREP platform.

Note: The content that you share to the PREP platform should not contain confidential information and you should not have any expectation of privacy with respect to it. You must exercise caution when submitting information that would be available to the specified user groups.

Your log in credentials or the log in credentials administrators create for their users are the designated facility’s manager of PREP’s responsibility. You are fully accountable for any use or misuse of your account and personal details as a result of conveying your details to someone else. You must maintain your log in details in absolute confidentiality and avoid disclosing them to those who should not have access. Make sure that you change your password frequently and at least one every six months.

 

Security

We take precautions to protect your information. When you submit sensitive information via the website, your information is protected both online (i.e., those with access to the PREP Platform) and offline (i.e., those with access to the PREP Platform, HAPevolve Staff). 

Wherever we collect sensitive information, that information is encrypted and transmitted to us in a secure way. You can verify this by looking for a lock icon in the address bar and looking for "https" at the beginning of the address of the Web page.

Remember, no method of transmission over the internet or method of electronic storage is 100% secure and reliable, and we cannot guarantee its absolute security.
